What exactly goes wrong through a job portal?
Skills, Languages and Certifications all live in the sidebar. A parser reading top to bottom, left to right, lands those fragments in the middle of your work history, so a recruiter searching the database sees a garbled record even though the PDF looks perfect.
Should I include the photo?
Only if you are applying in a photo-expecting market. Leave the field empty for the US, UK, Canada, Ireland, Australia and New Zealand — the sidebar closes up cleanly and the design still holds.
Does the photo sidebar count against me in a blind screening process?
It can. Organisations running anonymised sift will either strip the photograph or reject the format, and in the UK and US a photo on a CV is a routine reason to discard. Use this where photographs are expected, and send the Word export anywhere the process is anonymised.
